Distinctions are earned after doing something, then a day passes. After day 5, you can die, and anything done on that day is also included.

Butcher	Kill an animal, by throwing at a zombie, or butcher.	?
Home Improvements	Upgrade your house	No title to unlock
Excavations	Excavate in the wilderness	?
Clean	Days in the game without drugs (cumulative)	25x = Twinoid is taboo(?) 30x, but really it was only 14 days
Dead in your bed	Die in the town	No title to unlock
Home Depot Regular	Number of broken items that you've repaired	No title to unlock
