Distinctions are earned after doing something, then a day passes. After day 5, you can die, and anything done on that day is also included.

Butcher	Kill an animal, by throwing at a zombie, or butcher.	?
Home Improvements	Upgrade your house	?
Excavations	Excavate in the wilderness	?
Clean	Days in the game without drugs (cumulative)	?
Dead in your bed	Die in the town	?
